Questionnaire

A multi-step form/survey block (progress, choices, text input, previous/skip/next/submit) for chat-kit style conversational flows. Built on @shadcn/react, not react-aria-components — upstream hasn't ported it to the aria base yet. Experimental while @shadcn/react is pre-1.0.

Installation

    Register the @fma-ui namespace in components.json (once)

    Add the item

    pnpm dlx shadcn@latest add @fma-ui/questionnaire

    Files land in components/fma-ui/questionnaire.tsx; npm and registry dependencies are installed along with them.
